I can't see the reference range being used by the laboratory so I don't know how elevated these values are (assuming they are elevated above normal).

I assume these values are all elevated which generally indicate kidney disease. Most of the time this is due to renal failure but lymphoma in the kidneys could also cause the failure.

Most cats will benefit from a special diet meant for cats with renal disease, fluids (which may need to be intravenous initially or subcutaneous- your veterinarian will help you to decide), a phosphate binder and other medications if protein is being lost in the urine or if the blood pressure is high. If these values are very high (again- I can't see the reference range), then the prognosis is guarded but that's not to say that some pets won't respond to aggressive treatment.
