This is a very broad issue that can have many causes. The first step is to schedule a vet visit to rule out any health issues such as constipation, GI upset, or aging-related problems such as arthritis which can make it hard to get in and out of the litter box.
If there aren't any health issues, making sure the box is large enough for Coco to fully fit inside (without her bottom sticking over the edge), the litter is the right type she prefers, and that it and the surrounding area is clean are best. It's also a good idea to make sure the litter box is somewhere easy to access without other pets getting in the way, and not near food or water. And if Coco is consistently having accidents in one room or area, a second litter box in that spot may help her use it.
Lastly, an enzymatic cleaner such as Nature's Miracle is good to have on hand. It can break down any stool or urine particles where the accident was to help discourage going there again.
